Section 68IAC2-2-2. Application fees

  •    (a) All fees shall be submitted to the commission in the form of a certified check or cashier's check made payable to the state of Indiana.

      (b) An applicant for a supplier's license must pay a nonrefundable application fee in the amount of five thousand dollars ($5,000). The application fee must be submitted with the supplier's license application.

      (c) The application fee shall be utilized to conduct the background investigation of an applicant. If the cost of the background investigation relating to the applicant exceeds the application fee set forth in this rule, the executive director shall inform the applicant that it must pay an additional fee. Once an applicant is directed to submit an application fee in excess of the five thousand dollar ($5,000) fee set forth in this rule, the investigative team conducting the investigation on that applicant shall not finalize the report on the applicant's suitability for licensure nor submit that report to the commission for consideration until the applicant has paid the additional fee.
