5. Exempt entities under IC 22-3-2-5(a) shall provide notice to the board of their election. The name and contact information of a person authorized to act for the purposes of worker's compensation matters, along with other pertinent data, shall be provided on the form prescribed by the board for that purpose and updated annually. Details regarding excess coverage must be included.
