Section 460IAC6-19-5. Evaluation of available providers

(a) A provider of case management services shall provide the individual or the individual's legal representative, if applicable, with the following information:

(1) A current list of providers approved under this article, including a complete description of services offered by each provider.

(2) Information regarding services the individual may need that are not provided under this article.

(3) The current BDDS information guide for individuals on how to choose a provider.

(b) The provider of case management services shall assist the individual or the individual's legal representative, if applicable, in evaluating potential service providers.
