Section 856IAC1-33-3. Patient profile requirements

  •    The pharmacist shall assure that prescription medication profiles are maintained for all patients receiving pharmaceutical care at that pharmacy. Within limits of reasonably available information, the pharmacy medication profile shall include the following:

    (1) Name, address, telephone number, age or date of birth, and gender.

    (2) Known drug allergies and adverse reactions.

    (3) A list of current medications and relevant devices, either of which may relate to the patient's drug therapy.

    (4) Known disease states.

    (5) Any other information that, in the pharmacist's professional judgment, the pharmacist deems appropriate.

    (6) Pharmacist's comments relevant to the individual's drug therapy.
