Section 804IAC1.1-2-5. Issuance of certificate of registration

  •    (a) Upon successful completion of the requirements for registration and the board's approval of the application, the board shall:

    (1) order a certificate of registration to be issued to the applicant; and

    (2) designate the date of the certificate.

    Upon receipt of the registration fee, the board will assign a certificate number to the registrant. Thereafter, the board will issue a registration card and provide a certificate of registration.

      (b) If the board does not approve the application, the applicant shall be notified in writing of the grounds for its action. Notification shall be mailed to the last address furnished by the applicant.

      (c) Failure of any applicant to pay the issuance fee will cause the application to be terminated one (1) year after the board's action granting registration provided the board shall have sent a second notice to the applicant.

      (d) Each registrant shall keep the board advised of his or her current address.
