Section 680IAC2-3-21. Incident reports

When an incident occurs which serves to render a boiler or pressure vessel inoperative, the owner or user or authorized inspection agency responsible for the inspection of the boiler or pressure vessel shall immediately notify the chief inspector and submit a detailed report of the incident. In case of a serious incident such as an explosion, notice shall be given immediately by telephone, telegraph, or messenger and neither the boiler nor pressure vessel nor any of the parts thereof shall be removed or disturbed before an inspection has been made unless for the purpose of saving human life. The inspector shall submit a detailed report of the incident to the director. Such report shall include the cause and the origin of the incident, where determinable, and recommendations to prevent a recurrence of the incident. If major repairs are required, the inspector responsible shall also submit a repair report on the form approved by the division.
