Section 675IAC12-5-4. Application process

   (a) A variance from the rules of the commission may be applied for at any time.

  (b) An applicant for a variance shall do the following:

(1) Submit an application to the commission on a:

(A) form provided by the commission staff; or on a

(B) reasonable facsimile;

the information required by section 5 of this rule.

(2) Pay the fee required under 675 IAC 12-3-4 by a check or money order payable to the fire and building services fund.

  (c) An application may be submitted:

(1) by the applicant; or

(2) on the applicant's behalf by a representative.
