Section 615IAC1-2-5. Form of pleadings

(a) There are no specific requirements of form for any pleading or other paper except as provided for by this section.
(b) Cases, except those initiated by a Petition for Modification of Abatement, shall be titled:
Commissioner of Labor,
Complainant,
v.
(Name of party who petitioned for review)
Respondent.
(c) Cases initiated by a Petition for Modification of Abatement shall be titled:
(Name of Employer),
Petitioner,
v.
Commissioner of Labor,
Respondent.
(d) The titles provided for in subsections (b) and (c) shall appear at the upper left portion of the initial page of any pleading or paper (other than exhibits) filed.
(e) The initial page of any pleading or paper (other than exhibits) shall show opposite the title, the docket number, if known, assigned by the Board.
