Section 405IAC5-28-10. Chemotherapy

(a) Outpatient administration of chemotherapy and costs related to this therapy, including catheterization, physician's visit, cost of drug and solutions, pump regulators, and servicing, will be covered and do not require prior authorization.

(b) Chemotherapy services provided by a home health agency are subject to the prior authorization criteria at 405 IAC 5-16-3.
