Section 405IAC5-14-21. Maxillofacial surgery

   Providers shall be required, based upon the facts of the case, to obtain a second or third opinion substantiating the medically necessary service or approach for maxillofacial surgery related to diseases and conditions of the jaws and contiguous structures. The second opinion is required regardless of the surgical setting in which the surgery is to be performed, such as an ambulatory surgical treatment center, a hospital, or a clinic.
