Section 405IAC5-4-1. Enrollment of providers

  •    (a) In order to receive reimbursement under the Indiana Medicaid program, a provider must be enrolled to participate as a Medicaid provider. A provider is enrolled to participate in Medicaid when all of the following conditions have been met:

    (1) The provider is duly licensed, registered, or certified by the appropriate professional regulatory agency pursuant to state or federal law, or otherwise authorized by the office.

    (2) The provider has submitted an application to participate in the Indiana Medicaid program and completed such forms as may be required by the department.

    (3) The provider has signed and returned a Medicaid provider agreement.

    (4) The provider has received a provider number from the Medicaid contractor.

      (b) Out-of-state institutional or individual providers must be duly certified, licensed, registered, or authorized as required by the state in which the provider is located and must fulfill the conditions listed in subsection (a)(2) through (a)(4) before receiving any Indiana Medicaid payment.
