       (a) The commissioner may revoke the accreditation of an examination provider if the provider:

    (1) violates or fails to meet a requirement of this rule;

    (2) falsifies information on an application under section 2.1 of this rule to provide examinations for individuals taking the examination;

    (3) falsifies information about individuals taking the examination; or

    (4) fails to provide a quality examination based on documented and verified complaints registered with the commissioner or the department about an examination.

      (b) A revocation may be appealed under IC 4-21.5.
