Section 312IAC25-4-115.1. Post permit issuance information requirements

1. Within sixty (60) days of any addition, departure, or change in position of any person identified in sections 17 and 58 of this rule, the permittee must provide the following:

(1) The information required under sections 17(b)(1) through 17(b)(3) and 58(a)(3)(A) through 58(a)(3)(C).

(2) The date of any departure.
