Section 312IAC18-5-3. Laboratory fees and replacement certificates

(a) The division shall be reimbursed for the actual cost of expenses incurred in performing laboratory services.

(b) The fee for the replacement of a certificate, where changes are made by the person who requests the certificate, is ten dollars ($10).
