    DIGEST

    Temporarily adds rules to require the immunization of all children against hepatitis B and chickenpox. Authority: IC 4-22-2-37.1; IC 16-19-3-4; IC 16-41-1-2. Effective April 16, 2009.


    SECTION 1. The definitions in this document apply throughout this document.

    SECTION 2. "Child" means a person less than nineteen (19) years of age.

    SECTION 3. In addition to those immunizations required pursuant to IC 20-34-4 and 410 IAC 1-1, every child residing in Indiana shall be immunized against hepatitis B and chickenpox.

    SECTION 4. The child's age for administering each vaccine and the adequately immunizing doses shall be those currently recommended by the United States Public Health Service Advisory Committee on Immunization Practices (USPHSACIP).

    SECTION 5. The parents shall furnish to the governing body of the child's school corporation an adequately documented statement of the child's immunizations against hepatitis B and chickenpox.

    SECTION 6. Adequate documentation of immunization shall consist of:
    (1) a physician's certificate, if available; or
    (2) immunization records forwarded from another school corporation; or
    (3) a record maintained by the parent showing the month and year during which each dose of vaccine was administered.

    SECTION 7. SECTIONS 1 through 6 of this document take effect April 16, 2009.
