    DIGEST

    Amends 410 IAC 1-1-1 to update and modify the list of communicable diseases that require documentation of immunity. Amends 410 IAC 1-1-2 and 410 IAC 1-1-3 concerning statements of immunization histories and documentation requirements. Amends 410 IAC 1-1-4 concerning school reporting forms. Effective 30 days after filing with the Publisher.



    SECTION 1. 410 IAC 1-1-1 IS AMENDED TO READ AS FOLLOWS:

    410 IAC 1-1-1 Immunization requirements

    Sec. 1. Immunization requirements. Every child less than nineteen (19) years of age residing in Indiana shall be immunized against the following:
    (1) Diseases listed in IC 20-34-4.
    (2) Meningitis.
    (3) Varicella.
    (4) Pertussis (whooping cough).
    (5) Additional diseases for which immunizations are recommended by both the Indiana state department of health and the Centers for Disease Control and Prevention (CDC).
    For those diseases listed in IC 20-8.1-7 (diphtheria, tetanus, whooping cough, poliomyelitis, measles, and rubella), IC 20-34-4, and those diseases for which immunizations are recommended by the United States Public Health Service Advisory Committee on Immunization Practices (ACIP) and published by the CDC in the Morbidity and Mortality Weekly Report (MMWR), the adequately immunizing doses and the child's age and appropriate intervals for administering each vaccine shall be those recommended in the current Report of the Committee on Infectious Diseases of the American Academy of Pediatrics ("Red Book") or those currently recommended by the United States Public Health Service Advisory Committee on Immunization Practices. and published by the CDC in the MMWR.

    SECTION 2. 410 IAC 1-1-2 IS AMENDED TO READ AS FOLLOWS:

    410 IAC 1-1-2 Immunization record

    Sec. 2. Statement of Immunization History. When a child enrolls for the first time in a school corporation for the first time, and annually thereafter, if the child does not have a record of appropriate immunizations in the immunization data registry, as established by IC 16-38-5, the parents parent or legal guardian shall furnish provide to the governing body of that corporation an adequately documented statement a record of the child's immunizations which shall show that the child has received at least the minimum number of doses for his age as recommended by the official bodies named in Section 1. The statement shall also show whether the child has been tested for sickle cell anemia or for lead poisoning and the results of any such testing. signed by a physician or health care provider or an official immunization registry record.

    SECTION 3. 410 IAC 1-1-3 IS AMENDED TO READ AS FOLLOWS:

    410 IAC 1-1-3 Documentation of immunization history

    Sec. 3. Documentation. Adequate documentation of an immunization history shall consist of an immunization record:
    (a) a physician's certificate, if available; or
    (1) signed by a physician or health care professional;
    (b) immunization records forwarded (2) from another school corporation; a state immunization registry; or
    (c) a record maintained by the parent showing the month and year during which each dose of vaccine was administered.
    (3) from a school corporation.

    SECTION 4. 410 IAC 1-1-4 IS AMENDED TO READ AS FOLLOWS:

    410 IAC 1-1-4 School immunization reporting

    Sec. 4. Reports of (a) This section concerns school immunization and Testing reporting requirements to the Indiana state Board department of health. All reports required to be made to the State Board of Health by school corporations shall be submitted on forms prescribed and provided by the Board for those purposes.

    (b) Schools shall review and update all student immunization records annually.

    (c) All schools are required to report immunization data to the department, in compliance with IC 20-34-4-6, electronically through the use of the immunization data registry, as established by IC 16-38-5, annually.
