Agency Correction LSA Document #16-57(AC)

Under IC 4-22-2-38, corrects the following typographical, clerical, or spelling errors in the Indiana Administrative Code:

(1) In 329 IAC 13-3-1(m), delete "Government Printing Office, Superintendent of Documents, P.O. Box 371954, Pittsburgh, Pennsylvania 15250-7954, [phone]." and insert "Government Publishing Office, 732 North Capitol Street NW, Washington, D.C. 20401 or may be viewed at www.gpo.gov.".

(2) In 329 IAC 13-6-3(b), delete "To request EPA Form 8700-12, call the department at [phone].".

(3) In 329 IAC 13-6-3(b), delete "P.O. Box 7035, Indianapolis, Indiana 46207-7035." and insert "Regulatory Reporting Section, 100 North Senate Avenue, Room 1101, Indianapolis, Indiana 46204-2251.".

(4) In 329 IAC 13-7-2(b), delete "To request EPA Form 8700-12, call the department at [phone].".

(5) In 329 IAC 13-7-2(b), delete "P.O. Box 7035, Indianapolis, Indiana 46207-7035." and insert "Regulatory Reporting Section, 100 North Senate Avenue, Room 1101, Indianapolis, Indiana 46204-2251.".

(6) In 329 IAC 13-7-3(b)(7)(D)(i), delete "Office of Land Quality, Indiana Department of Environmental Management, 100 North Senate Avenue, Indianapolis, Indiana 46206-6015, [phone]." and insert "Indiana Department of Environmental Management, Office of Land Quality, Emergency Response Section: [phone] for in-state calls (toll free), or [phone] for out-of-state calls.".

(7) In 329 IAC 13-8-3(b), delete "To request EPA Form 8700-12, call the department at [phone].".

(8) In 329 IAC 13-8-3(b), delete "P.O. Box 7035, Indianapolis, Indiana 46207-7035." and insert "Regulatory Reporting Section, 100 North Senate Avenue, Room 1101, Indianapolis, Indiana 46204-2251.".

(9) In 329 IAC 13-9-4(b), delete "To request EPA Form 8700-12, call the department at [phone].".

(10) In 329 IAC 13-9-4(b), delete "P.O. Box 7035, Indianapolis, Indiana 46207-7035." and insert "Regulatory Reporting Section, 100 North Senate Avenue, Room 1101, Indianapolis, Indiana 46204-2251.".
