TITLE 760 DEPARTMENT OF INSURANCE

Agency Correction LSA Document #14-64(AC)

Under IC 4-22-2-38, corrects the following typographical, clerical, or spelling errors in the Indiana Administrative Code:

(1) In 760 IAC 1-15.1-7.2, FORM F, ENTERPRISE RISK REPORT, delete the line immediately below "Name of Registrant/Applicant".

(2) In 760 IAC 1-15.1-7.2, FORM F, ENTERPRISE RISK REPORT, below "Name of Registrant/Applicant", delete "Name of Other Person Involved in Merger or Acquisition".

(3) In 760 IAC 1-15.1-7.2, FORM F, ENTERPRISE RISK REPORT, below "On behalf of/related to the following Insurance Companies", insert "Name" on the left margin and "Address" on the right margin.

(4) In 760 IAC 1-56-10(d)(4), after "underlying reinsurance agreement in existence prior to December 31,", delete "2012" and insert "2013".

(5) In 760 IAC 1-56-10(d)(4), after "will continue to be acceptable until December 31,", delete "2013" and insert "2014".
