TITLE 410 INDIANA STATE DEPARTMENT OF HEALTH

Agency Correction LSA Document #10-62(AC)

Under IC 4-22-2-38, corrects the following typographical, clerical, or spelling errors in the Indiana Administrative Code:

(1) In 410 IAC 5-1-2, delete "Indiana state board of health" and insert "Indiana State Department of Health".

(2) In 410 IAC 5-1-10, delete "Indiana State Board of Health, 1330 West Michigan Street, Indianapolis, Indiana 46206" and insert "Indiana State Department of Health, 2 North Meridian Street, Indianapolis, Indiana 46204".

(3) In 410 IAC 5-3-25, delete "Indiana state board of health [This language does not appear, as stated, in 410 IAC 5-3-25.]" and insert "Indiana State Department of Health".

(4) In 410 IAC 5-3-27, delete "INDIANA STATE BOARD OF HEALTH [This language does not appear, as stated, in 410 IAC 5-3-27.]" and insert "INDIANA STATE DEPARTMENT OF HEALTH".

(5) In 410 IAC 5-3-30, delete "Radiological Health Department [Section], Indiana State Board of Health" and insert "Medical Radiology Services Division, Indiana State Department of Health".

(6) In 410 IAC 5-3-31, delete "Radiological Health Department [Section], Indiana State Board of Health" and insert "Medical Radiology Services Division, Indiana State Department of Health".

(7) In 410 IAC 5-3-32, delete "Radiological Health Department [Section], Indiana State Board of Health, 1330 West Michigan Street, Indianapolis, Indiana 46206" and insert "Medical Radiology Services Division, Indiana State Department of Health, 2 North Meridian Street, Indianapolis, Indiana 46204".
