Section 915IAC2-3-4. Resident services agreement

   The resident services agreement is a contract that shall be signed by the applicant or resident and shall include, but not be limited to, the following:

(1) The name and address of the applicant or resident and his or her legal representative, if any.

(2) The detail of the applicant or resident's income and resources used to calculate the maintenance charges.

(3) The amount of the maintenance charge for the applicant or resident.

(4) A statement that the applicant or resident agrees to comply with the following:

(A) All home policies regarding residents.

(B) All applicable state laws.

(C) This article.
