  •    An application for a club permit shall include a membership list with the names and addresses of each member of the club and of any affiliated organization auxiliary to the club who are permitted to use the facilities licensed by the requested permit.

      Said membership lists shall be amended and revised by the permittee, showing cancellations of membership and new members, every six months.
