Section 856IAC2-3-27. Modification of registration

  •    Modification in registration. Any registrant may apply to modify his registration to authorize the handling of additional controlled substances or to change his name or address by submitting a letter of request to the Indiana Board of Pharmacy. The letter shall contain the registrant's name, address, registration number, and the substances and/or schedule to be added to his registration or the name or address and shall be signed by the same person who signed the most recent application for registration or re-registration. If the registrant is seeking to handle additional controlled substances listed in Schedule I [856 IAC 2-2-2] for the purpose of research or instructional activities, a Federally approved research protocol describing each research project involving the additional substances shall be subject to inspection by the Indiana Board of Pharmacy or he shall attach two copies of a statement describing the nature, extent, and duration of such instructional activities, as appropriate. No fee shall be required to be paid for the modification. The request for modification shall be handled in the same manner as an application for registration.

      If the modification of registration is approved, the Indiana Board of Pharmacy shall issue a new certificate of registration to the registrant, who shall maintain it with the old certificate of registration until the expiration date.
