Section 856IAC1-15-1. Pharmacist leaving employ of pharmacy; notice to board; application to qualify permit

  •    If a qualified pharmacist, who, having upon the basis of his or her qualifications caused a pharmacy permit to be granted to any person, firm, corporation or copartnership desiring to operate, maintain, open or establish a pharmacy should leave the employ of such pharmacy, he or she shall immediately notify the Indiana board of pharmacy (board) and the owner shall file an application with the board to qualify the permit with another pharmacist.
