Section 840IAC1-1-12. Duplicate licenses

  •    (a) Upon receipt of satisfactory evidence that a license has been lost, stolen, mutilated, or destroyed, the board may issue a duplicate license upon:

    (1) compliance with conditions as the board may prescribe; and

    (2) payment of a fee as determined by the board.

      (b) A practitioner may hold both an HFA and RCA license provided that all requirements of this rule are met, including separate applications to be submitted with the appropriate fees for each license application and renewal.
