Section 760IAC2-8-2. Any other health insurance policies

Latest version.

Insurance producers shall list the following:

(1) Any other health insurance policies they have sold to the applicant.

(2) Policies sold which are still in force.

(3) Policies sold in the past five (5) years that are no longer in force.
