On or before March 1 of each year, an insurer, a health maintenance organization, and a limited service health maintenance organization must submit electronically to the department a grievance procedure report for the preceding calendar year on the form set forth in section 14 of this rule. A health maintenance organization and a limited service health maintenance organization may submit the information required by IC 27-13-8-2(a)(2) and IC 27-13-8-2(a)(3) concurrent with this filing.
