Section 760IAC1-49-11. Fees

(a) The fee for initial application for certification as a medical claims review agent is one hundred fifty dollars ($150) and must accompany the application.

(b) The annual renewal fee for a certificate as a medical claims review agent is one hundred dollars ($100) and must accompany the application. The annual renewal fee is nonrefundable.
