Section 560IAC2-7-4. Cover sheet

Latest version.

Any document sent to the Indiana education employment relations board or its agent by electronic facsimile transmission shall be accompanied by a cover sheet which states the following:

(1) Title of the document.

(2) Case number.

(3) Number of pages.

(4) Identity and voice telephone number of the sending party.

(5) Instructions for filing.

The cover sheet shall contain the signature of the attorney or party authorizing the filing.
