Section 511IAC10.1-3-9. Annual report by institution

   (a) Each year, the department will provide institutions with the format for the annual program report integrated within the annual accreditation report by a date determined by the department.

  (b) An institution shall submit an annual program report to the department, including the number of participants who have:

(1) enrolled and are participating only in the preparation program; and

(2) completed the preparation program.
