  •    "Interdisciplinary team" means those individuals who have responsibility for making decisions about and implementing individual program plans. The team may include, but is not limited to, the resident, his or her legal guardian, any representative of the resident's choice, case manager, and representatives from agencies who provide a direct service for the resident.
