Section 460IAC7-5-7. Individualized support plan participants section

Latest version.

   (a) The individualized support plan participants section shall list each person participating in the development of the ISP.

  (b) The relationship of each participant to the individual shall be indicated.

  (c) The date or dates the ISP was forwarded to each participant shall be indicated.

  (d) The method by which the ISP was forwarded to the participant shall be indicated.
