Section 460IAC6-5-21. Therapy services provider qualifications

  •    (a) To be approved to provide therapy services, an applicant shall be:

    (1) a psychologist licensed under IC 25-33-1 and have an endorsement as a health service provider in psychology pursuant to IC 25-33-1-5.1(c);

    (2) a marriage and family therapist licensed under IC 25-23.6;

    (3) a clinical social worker licensed under IC 25-23.6; or

    (4) a mental health counselor licensed under IC 25-23.6.

      (b) For an entity to be approved to provide therapy services, the entity shall certify that, if approved, the entity will provide therapy services using only persons who meet the qualifications set out in this section.
