  •    (a) If the individual referred to the provider is determined through the assessment to be eligible for adult guardianship services, the provider shall formulate and adopt an individualized service plan that provides the least restrictive service.

      (b) The service plan shall describe each of the following:

    (1) The individual's primary difficulties.

    (2) The reason the individual was referred to the provider.

    (3) The services needed and referrals made to obtain those services.

    (4) Short term and long term objectives.

    (5) Rationale for services.
