  •    (a) To obtain a license to operate a birthing center, an application for a license to operate a birthing center must be submitted to the division. The application may be submitted simultaneously with the request for plan review before construction. At the latest, however, the application must be received by the department at least forty-five (45) days before the anticipated opening of the center. At least forty-five (45) days before the opening of the center, the applicant must inform the division of the anticipated date of opening.

      (b) The initial license application includes the following:

    (1) An application for a license to operate a birthing center on a form prescribed by the division.

    (2) Documents required by the application for a license to operate a birthing center.

    (3) The appropriate license fee.

      (c) All changes in ownership, name, and address must be reported in writing to the division. Reapplication must be filed when a change of fifty percent (50%) or greater ownership occurs.
