Section 410IAC18-4-2. Documentation required to obtain death certificate

  •    The applicant of a death certificate shall provide the following to the state registrar upon request:

    (1) Valid identification.

    (2) Other documentation as requested by the state registrar.

    (3) A check or money order payable to the Indiana state department of health for the correct amount required by law.
