Section 410IAC1-1-2. Immunization record

   When a child enrolls in a school corporation for the first time, and annually thereafter, if the child does not have a record of appropriate immunizations in the immunization data registry, as established by IC 16-38-5, the parent or legal guardian shall provide to the governing body of that corporation a record of the child's immunizations signed by a physician or health care provider or an official immunization registry record.
