Section 405IAC5-36-2. Requirements for the provision of DSMT

Latest version.

   (a) DSMT must be medically necessary for the patient.

  (b) DSMT must be ordered in writing by a physician or podiatrist licensed under applicable Indiana law.

  (c) DSMT must be provided by a health care professional licensed under applicable Indiana law.

  (d) The health care professional that provides DSMT must have specialized training in the management of diabetes.
