Section 405IAC5-1-6. New or experimental product, service, or technology

   (a) A provider may request consideration for coverage of any new or experimental product, service, or technology not specifically covered in this article. Such a request must be submitted by the provider to the office along with a detailed written statement, along with all available supporting documentation, justifying how such product, service, or technology is medically necessary.

  (b) This section does not apply to legend drugs.
