Section 345IAC5-2-39. "Permit" defined

  •    "Permit" means an official document issued by the USDA, the state veterinarian, or an accredited veterinarian for the movement of animals. A permit must be issued at the point of origin, must be reissued for each change in destination, and must contain the following information:

    (1) The name and address of the animal's owner.

    (2) The origin and destination of the animal.

    (3) The number of animals covered.

    (4) The purpose of the movement.

    (5) Whether the animal is from an exposed, noncompliant, infected, or source flock or a flock under investigation.

    (6) Whether the animal is a high-risk, exposed, scrapie-positive, or scrapie-suspect animal.

    (7) All forms of official identification on each animal.

    (8) The transportation vehicle license number or other identification number.

    (9) If the shipment is sealed, the seal number.
