Section 326IAC18-1-10. Duplicate asbestos license

   (a) In order to replace an asbestos license which has been lost or stolen, a person shall submit a completed application for a duplicate license on a form provided by the department.

  (b) The form shall include a statement indicating that the original asbestos license was lost or stolen.

  (c) The department shall issue no more than two (2) duplicate asbestos licenses to any person in any calendar year.
