Section 312IAC5-4-2. Application for a license for a navigation aid or water recreation structure; general requirements

An application for a license under this rule must be completed on a department form and contain information sufficient to describe the purpose for which the license is sought. The application shall include the following information:

(1) The name and address of the applicant.

(2) The name, address, and telephone number of an individual who can be contacted concerning placement, maintenance, and removal of the navigation aid or water recreation structure.

(3) A description of any navigation aid or water recreation structure to be placed under the license.

(4) The date, location, and duration of placement.
