Section 11IAC2-2-2. Required information for consumer registration

Any method of registering with the telephone privacy list shall require the consumer to provide, at a minimum, the following information:

(1) The consumer's name.

(2) The residential telephone number or numbers the consumer wishes to register with the telephone privacy list.

(3) The consumer's address.
